Information for Pharmacists

 

Qsymia is only available through certified pharmacies. Certified pharmacies must follow the requirements of the Qsymia REMS Pharmacy Program.

Steps for Pharmacy Certification:
  1. Successful completion of Pharmacy Training Program and Knowledge Assessment.
  2. Completion of pharmacy enrollment online via website or by faxing the enrollment form into the program contact center at 1-855-302-6699.
You must enroll in the Qsymia REMS Pharmacy Program to dispense Qsymia.

If you are already enrolled in the Qsymia REMS Pharmacy Program but do not have a web user account, please click on the Create My Account button to get started. You will be asked to provide your Enrollment ID along with creating a username and password to access your enrollment information.
